This report provides additional findings from the 2021 Request for Information (RFI) survey about the current characteristics of community-based organization (CBO) contracts with health care partners and illustrates the ways in which those arrangements are expanding services, reaching high-risk clients, focusing on social determinants of health (SDOH), and tailored to the core strengths of the CBO and the priorities of the health care partner.
