Step 1: Choose Your Affiliation Method

We offer two affiliation methods when applying for disability services: Standard Affiliation and Accelerated Affiliation.

Standard Affiliation (Meeting Required with an Access Coordinator)

This method is designed if you want to have a 45-60-minute access consultation over Zoom, or in person with an access coordinator to review your documentation and create an accommodation plan together. This method is a good fit if you are unsure which accommodations would be most supportive, or have more nuanced access needs and would like to meet to further discuss. During this meeting, your access coordinator will ask several questions to learn more about you, determine what types of barriers may exist, learn about your personal experiences with your diagnosis and identity what accommodations are reasonable and appropriate.

Oxford Students Only: Accelerated Affiliation (No Meeting Required with an Access Coordinator)

This method is designed if you want to only explore the specific accommodations listed below at this time and you have documentation. This method may be a good fit if you know what accommodations you would like to explore or have previous experience with academic accommodations, and those accommodations are on the list below. Within this method, communication primarily occurs via email, there is no access consultation meeting required, and accommodation requests will be reviewed within 3 business days.

Choosing the accelerated affiliation option does not limit you to the listed accommodations indefinitely. If your access needs change in the future and you would like to explore additional accommodations, you can connect with your access coordinator.

Accelerated Affiliation Accommodations

  • Testing accommodations: Extended test time: 1.5x or 2x, small group distraction reduced environment
  • Note taking accommodations: Ability to audio record lectures, advanced access to classroom materials, access to an electronic device for note taking
  • Classroom accommodations: Breaks as necessary, access to food/beverage/medical device
  • Scheduling accommodations: Time of day accommodations, reduced course load

Step 2: Submit an Application

Once you have selected which affiliation method best fits your needs submit your application using the following links.

Step 3: Monitor Your 兔子先生 Email

After you have submitted your affiliation application, please monitor your 兔子先生 email for communications from our front office and your access coordinator about the status of your application and next steps.

Standard Affiliation: If you are pursuing a standard affiliation, you will receive email communication from the Miller Center in the event your application does not have documentation and it is needed to move forward in the accommodation determination process. You will also receive an email with the subject "Schedule an Access Consultation" from your assigned access coordinator with a link to schedule an appointment after review.

Accelerated Affiliation: If you are pursuing an accelerated affiliation you will receive email communication from the Miller Center once your application has been reviewed. If accommodations are determined to be reasonable through the accelerated affiliation process, you will receive an email with instructions and next steps. If the application or documentation does not provide the necessary information to determine reasonable accommodations through the Accelerated Affiliation process, you will receive an email with steps to schedule an Access Consultation.

Important Considerations

Please review the following information prior to submitting an application.

  • If you require more than 30 minutes to complete the application, we recommend typing your responses in a separate document before submitting the application to avoid the system timing out.
  • You will receive a confirmation email to your 兔子先生 email account immediately following submission. Included in this message is a link to your application that can be used to submit documentation to at a later point.
  • Standard affiliation applications are reviewed within approximately 10 business days. You will receive notification regarding your application status to your 兔子先生 email account. This notification will include instructions regarding the next steps that should be taken to complete formal affiliation.
  • Documentation delivered directly to SDS by you or your healthcare providers will be uploaded by SDS staff to your SAM profile.
