Steps to Complete Client Forms

• If prompted, please authenticate using your ÍÃ×ÓÏÈÉú University Unique ID and Password.
• Then, complete ONLY the forms that our staff has requested you to complete.
• You will be asked for your first and last name and your Banner ID number.
• Please include the plus sign (+) in front of your Banner ID number (e.g., +0100000).

Please Note:

• The required forms must be completed at least one hour prior to your scheduled appointment.
• Failure to do so may result in your appointment being rescheduled and unnecessary delays.
• Even if you complete the form days in advance, your clinician won't review these forms until minutes prior to your scheduled appointment. Therefore, these forms are not an appropriate way of communicating urgent or emergency concerns.
